Transforming Elderly Care Through Community and Tech
Fostering community bonds of carers
This report documents the design and development of Snap & Go, a user-centered app created to address the challenges faced by elderly individuals and their carers in Sai Kung District villages. Drawing on extensive research from 2022 to 2024, it explores how a digital tool can mobilize neighborhood support networks, alleviate caregiver stress, and strengthen community connections in rural areas.

This report documents the redesign of CarerEPS, a user-centered platform aimed at supporting carers across Hong Kong. Developed under the “Community Support for Carers” initiative by J.C.DISI and HKCSS, it builds on extensive research and co-creation from 2022 to 2024. The project explores how a digital tool can ease the burdens on carers, enhance access to critical resources, and foster stronger community connections, offering a vital lifeline to those navigating caregiving challenges in an increasingly digital society.
